What are Chronic Diseases?

Chronic diseases such as arthritis, diabetes, heart disease and cancer are the leading causes of death and disability in the Canada. Many preventable chronic diseases are caused by a short list of risk behaviors: including physical inactivity, smoking, poor nutrition, and excessive alcohol use. Physiotherapy is an important aspect of chronic disease management aimed at treating current symptoms and preventing future health problems. Canadians are living longer and spending more years in good health. In 2021, life expectancy at birth was 81.6 years overall with males living an average of 79.3 years and females living an average of 84.0 years. However, 45.1% of Canadians lived with at least one major chronic disease in 2021. Some of the most common chronic conditions in Canada in 2021 were overweight (35.5%), obesity(29.2%), arthritis (19.5%) and high blood pressure (17.7%).

What is Chronic Care Management?

Chronic Care Management is a low-cost program that helps adults with chronic diseases learn how to manage and improve their health. Topics covered include pain management, healthy eating, exercise and physical activity, medication use, mental health, and communicating with healthcare professionals.

 

Fifteen hours of content are covered during weekly sessions held over a six-week period with disease-specific content for CHRONIC PAIN, ARTHRITIS, OSTEOPOROSIS AND DIABETES. Throughout the program, participants are guided to focus on building skills to manage their conditions.
